On 12 March 2009, a Sikorsky S-92A being operated by Cougar Helicopters' on a passenger flight from St. John's, Newfoundland Canada to the Hibernia offshore oil platform declared an emergency and began a return after total loss of oil from the main gear box but ten minutes later, control was lost during an attempted ditching and all but one occupant died. The Investigation attributed the loss of lubrication to sheared bolts securing the MGB which had allowed all the oil proving lubrication to escape.
