This report has been developed among a series within the Human Error in ATM (HERA) Project dealing with how error in Air Traffic Management (ATM) can be analysed and evaluated to improve safety and efficiency in European ATM operations. The purpose of this work is to develop an approach, using the Human Error in ATM (HERA-JANUS) classification technique (see EATMP, 2003), for safety management within ATM. The report describes the developments made of the ‘Safety Management Assistance and Recording Tool (SMART)' approach over an eighteen-month period, and details the results and recommendations from this process.
