On 31 October 2019, an Airbus Helicopters EC225LP departing a helipad at Dokdo at night crashed into the sea after the Pilot lost situational awareness and failed to respond to last minute alerts from both the travelling engineer and the Co-Pilot to the clearly unintended descending flight path. All seven occupants were killed and the helicopter was destroyed. The absence of a departure briefing was assessed as specifically contributory but the main findings as to the underlying context for the accident were extensive procedural inadequacies in respect of the operational safety regime under which emergency medical flights were being conducted.
