On 5 March 2013, an Airbus A330-300 with an augmenting crew member in the flight deck was severely damaged during a tailstrike as it rotated for take-off from Chicago. The cabin crew reported an unusual noise but after a short review it was decided to complete the scheduled flight to Munich. On arrival structural damage sufficient to render the aircraft unfit for flight and unfit for pressurised flight even after temporary repair was discovered. The Investigation concluded that a combination of factors including a significantly higher rotation rate than recommended by Airbus had caused the tailstrike.
