The German BFU has made a study into the very large numbers of smoke/fumes occurrences in an attempt to establish how many are really Annex 13 events i.e. significant safety occurrences justifying independent investigation and how best to achieve meaningful investigations of such occurrences and safely ignore the majority of operational events which are not 'Serious Incidents'.
